All it takes to begin using our senior care services is to meet with us and verify your eligibility with a few necessary documents — and we take care of the rest.
Eligibility Requirements
To participate in our senior care services, you'll need to meet three requirements:
- Age: 65 or older
- Residence: Delaware resident
- Annual income ceiling:
- Single = $23,800
- Married = $36,600 (regardless of spousal eligibility)
Application Process
When applying for our senior care services:
- One of our staff will review your proof of age, residence and income
- We will ask for your insurance card or any insurance information you have
- We will also ask for contact information such as address, phone, emergency numbers, primary care physician and preferred pharmacy location
- If approved, you’ll be registered in our system and your membership will be good for one year. Eligibility must be determined and renewed annually.
